From what you say the details for a 3521.30.00 are on the Omega website.
However this number that you list MUST be for a silver dialed watch.
How do I know this. it's in the PIC code.

As you can see a blue dialed watch MUST have xxxx.80.xx as it's pic code. So it looks like your hangtag/PIC code is correct.
As for which bracelet came with the watch or date of manufacture, I have no idea.
